Transition ServicesOur Transition Services team will help you prepare to take your child home after a transplant. Transition Services will help you plan your child’s care at home, teach you specific skills that you will need as your child’s caregiver and provide referrals for home care, if necessary. Transition Services can help in the following ways:
Your “Parent & Caregiver Resource Manual” has detailed sections on care giving, long-term follow-up, managing your child’s medications and more. Be sure to ask if you have questions about any of the information in this manual. Contact Transition Services at (206) 288-2125.
