Partnering With Our Patients To Make Health Care Safer

Providing safe, quality care is a team effort. Research has shown that patients who are more involved with their care tend to see better results. Click here for more information.

Here is how you and your family can participate in your care to help reduce the risk of medical errors:

  • Be involved in your care.
  • Ask family members and friends to help with your care.
  • Know about your medicines and carry a list of all your current medications you are taking.
  • Know your medical history and treatment plans.
  • Help prevent the spread of infection.
  • Report concerns about your care

Patient Rights and Responsibilities

As a patient at SCCA, you have rights and share responsibility for your care. Click here for a copy of SCCA’s Patient Rights and Responsibilities information. Please let your providers know if you have concerns or complaints about any aspect of your care. We encourage you to talk with your health care team initially. Your nurse or social worker can be an advocate for you. You can also report your concerns online by clicking here.

Speak Up Campaign

In March 2002, The Joint Commission, together with the Centers for Medicare and Medicaid Services (CMS), launched a national campaign to urge patients to take a role in preventing health care errors by becoming active, involved and informed participants on the health care team. Click here for a free brochure on the “Speak Up” information.
