Transition Services


Our Transition Services team will help you prepare for transitions throughout your transplant process. Transition Services will help you plan your care at home, teach your caregiver specific skills that he or she will need and provide referrals for care, and home infusions, if necessary.

The transition from the hospital to Seattle Cancer Care Alliance clinic and to home can be stressful, and a time of high anxiety. But Transition Services can help you make this transition smoothly.

Transition Services can help in the following ways: 

  • By meeting with you, your family, and your caregiver to discuss your care at home before you are discharged from the hospital 
  • By teaching classes for caregivers on taking care of you at home 
  • By talking with the doctor who referred you to us about your medical needs and follow-up care 
  • By providing emotional support to you and your caregiver as you prepare to return home

Your “Patient & Caregiver Resource Manual” has detailed sections on care giving, long-term follow-up, managing your medications and more. Be sure to ask if you have questions about any of the information in this manual.
