To keep improving care, we ask you to stay in touch with us after your transplant. This helps our newest transplant patients get the best possible care. It also helps us learn how to provide more effective long-term follow-up, which may benefit you directly. What we track The questionnaire asks about all aspects of your health. For instance, it asks about a wide range of possible effects in your body from the transplant or transplant-related treatment. It also asks about issues related to the quality of your life, such as whether you have had any difficulties with your health insurance or any cognitive changes that may relate to your transplant.
