Transition Services
Our Transition Services team will help you prepare to take your child home after a transplant. Transition Services will help you plan your child’s care at home, teach you specific skills that you will need as your child’s caregiver and provide referrals for home care, if necessary.
The transition from Seattle Cancer Care Alliance (SCCA) to home can be a stressful one. This can be a time of high anxiety for parents, but Transition Services can help you and your child make this transition smoothly. “It’s like leaving the nest to make this transition from the center to home,” says Dr. Jean E. Sanders, MD, director of pediatric clinical hematopoietic cell transplantation at SCCA and Seattle Children’s.
Transition Services can help by:
- Meeting with you to discuss your child’s care at home during the first year post-transplant
- Teaching classes for you on taking care of your child at home
- Talking with your child’s pediatrician (or other referring doctor) about your child’s medical needs and follow-up care
- Providing emotional support to you as you prepare to assume the role of caregiver in the home setting
Your “Parent & Caregiver Resource Manual” has detailed sections on caregiving, long-term follow-up, managing your child’s medications and more. Be sure to ask if you have questions about any of the information in this manual.
